        【摘要】:但心臟電解剖標(biāo)測系統(tǒng)須逐點標(biāo)測,較費時,且標(biāo)測導(dǎo)管價格較貴,我們認為普通室上性心動過速患者,不適宜首選心臟電解剖標(biāo)測系統(tǒng)進行消融,但對于在復(fù)發(fā)、疑難房室結(jié)雙徑路和旁道及房性、室性心律失常病例,尤其有心內(nèi)結(jié)構(gòu)異常時,心臟電解剖標(biāo)測系統(tǒng)對靶點消融有很大指導(dǎo)作用。臨床研究已初步顯示了這些系統(tǒng)的可行性,可能是未來導(dǎo)管消融發(fā)展的方向之一。

        心臟電解剖標(biāo)測系統(tǒng)對心律失常的標(biāo)測和消融是國際上電生理領(lǐng)域中熱點,并在臨床上取得很好的治療效果,將射頻消融技術(shù)推向更深、更高的層次,臨床應(yīng)用中已顯示出明顯的優(yōu)越性。但心臟電解剖標(biāo)測系統(tǒng)須逐點標(biāo)測,較費時,且標(biāo)測導(dǎo)管價格較貴,我們認為普通室上性心動過速患者,不適宜首選心臟電解剖標(biāo)測系統(tǒng)進行消融,但對于在復(fù)發(fā)、疑難房室結(jié)雙徑路和旁道及房性、室性心律失常病例,尤其有心內(nèi)結(jié)構(gòu)異常時,心臟電解剖標(biāo)測系統(tǒng)對靶點消融有很大指導(dǎo)作用。CARTO系統(tǒng)功能較為完善,通過CartoMerge技術(shù)可更準(zhǔn)確和全面地展示心臟的解剖結(jié)構(gòu),提高導(dǎo)航精確性,增加手術(shù)的安全性及成功率。非接觸標(biāo)測系統(tǒng)EnSite Array對非持續(xù)性、血流動力學(xué)不能耐受的心動過速及不穩(wěn)定的心律失常具有獨特的優(yōu)勢。但心臟電解剖標(biāo)測系統(tǒng)需要手動操作,需要術(shù)者屈伸及旋轉(zhuǎn)導(dǎo)管到達目標(biāo)解剖位置進行定位消融,不僅操作難度大,而且學(xué)習(xí)曲線較長,依然會對醫(yī)師和患者造成放射性損傷。近年來新出現(xiàn)的磁導(dǎo)航指導(dǎo)下的電生理消融導(dǎo)管以及機械手操控的消融設(shè)備,將以往復(fù)雜的導(dǎo)管操控技術(shù)簡單、直觀化,可自動引導(dǎo)導(dǎo)管到達一些人工操作時難以到達的部位,消融療效可靠,同時縮短醫(yī)師消融手術(shù)的學(xué)習(xí)曲線。臨床研究已初步顯示了這些系統(tǒng)的可行性,可能是未來導(dǎo)管消融發(fā)展的方向之一。
