如果有證據(jù)顯示梗死相關(guān)血管持續(xù)閉塞或開通后再閉塞，如下降的ST段再次抬高等提示患者發(fā)生再梗死，應(yīng)該血管造影并根據(jù)情況進(jìn)行血運重建治療或轉(zhuǎn)運至可行PCI的醫(yī)院。如不能迅速（癥狀發(fā)作后60min內(nèi)）進(jìn)行PCI，可考慮進(jìn)行再次溶栓治療，并選擇無免疫原性的溶栓藥物。

雖然PCI在冠心病治療中應(yīng)用越來越廣泛，但是溶栓治療具有快速、簡便、經(jīng)濟(jì)、易操作的特點，仍然是減少STEMI患者病死率和改善預(yù)后的重要方法。選擇恰當(dāng)?shù)倪m應(yīng)證，減少出血并發(fā)癥，在最短的時間內(nèi)溶解血栓開通血管治療仍然具有不可替代的價值。溶栓藥物種類較多，應(yīng)首選纖維蛋白特異性的溶栓藥物。同時需要規(guī)范地進(jìn)行輔助抗血小板和抗凝治療，可大大提高血管開通率。在臨床實踐中應(yīng)該根據(jù)具體情況恰當(dāng)選擇適應(yīng)證，科學(xué)的進(jìn)行溶栓治療。
